What is concierge medicine?
Concierge medicine is a personalized healthcare model where patients pay a membership fee for direct access to a physician, longer appointment times, same-day/next-day availability, and comprehensive care—often including home visits. Unlike traditional insurance-based practices, concierge medicine focuses on quality over quantity.
Do you have access to specialists?
Yes! We have access to specialists all around the area. We are not associated with a large medical system like Baptist, so we can refer to all specialists! If needed, we can get you to see the specialist you need quickly. Dr. Mikals will personally call the specialist to make sure they are aware of your medical situation. Additionally, Dr. Samantha Mikals is an ENT and can evaluate and manage many ENT problems and spend the time you need to discuss your ENT concerns.
Do you take health insurance?
No, we do not accept health insurance. We operate on a direct-pay mode, which allows us to spend more time with patients, reduce administrative burdens, and provide highly personalized care without insurance restrictions. Insurance does not cover home visits in almost all situations. Medicare is able to cover home visits if a patient needs an ambulance transport to get to the clinic because they require a stretcher.
Do I still need health insurance?
Yes, it is very important you still have health insurance. Health insurance is still necessary to cover certain medications, imaging studies, specialty doctor visits, Emergency Department visits, and hospitalizations.
Is there a fee for this service?
There is a monthly or annual membership fee, which you pay upon enrollment. Annual membership is discounted compared to monthly membership. Please schedule a meet & greet with Dr. Mikals to find out more. Sign-up now
Are there additional charges after my membership fee?
Certain laboratory tests can be billed to your insurance, however many are much cheaper at the wholesale cash-pay rate. Certain medications or treatments, such as iron infusions or migraine botulinum toxin injections, would cost an additional fee. We do not bill your insurance. This allows Dr. Mikals to spend more time with patients.
Does insurance cover my membership fee?
Insurance does not cover your membership fee. Starting in 2026, Health Savings Accounts (HSA) will cover up to $150 per month for an individual or $300 for a family for direct primary care memberships, such as Jacksonville Concierge Medicine’s monthly membership.

What age patients do you see?
Dr. Kyle Mikals (Internal Medicine) can see patients 17 years or older. Dr. Samantha Mikals (ENT) can see patients of all ages. We are working on adding a Pediatrician to our practice.
How do I find your Aesthetics Studio?
Our Aesthetics Studio is located in the Parlor Suites at 7510 Gate Pkwy, Suite 201, #214. The entrance to the elevator lobby is between the entrances to 14 Prime and Sidecar, next to the “7510” sign on the building. We recommend you park near the green turf in front of V Pizza and Sidecar for quick access.

What aesthetic treatments do you offer?
Dr. Samantha Mikals curates aesthetic treatments that are easy to tolerate and achieve natural-looking results. She currently offers Letybo botulinum toxin injections with flat fee pricing and Density radiofrequency collagen stimulation treatments.
How long does a Density treatment take?
A typical face and neck treatment takes about 90 minutes. If you’re treating additional areas, it would take a bit longer.
Is there any downtime with Density?
Most people return to normal activities immediately after treatment. Some may experience slight redness or warmth that fades within hours. The main thing is to avoid overheating your skin for about 24 hours after Density treatment.
What does a Density treatment feel like?
You’ll feel a gentle warmth during treatment, balanced with cooling technology to keep your experience comfortable. Your provider will adjust the treatment to your comfort level. Many people consider Density to be more comfortable than any other RF treatments on the market. Some have described it as relaxing!
When will I see results from my Density treatment?
Many people see a noticeable difference within days, with continued improvement over 3–6 months as your skin produces new collagen. Some people noticed continued results for 12-18 months!
How many Density treatments do I need?
Most people note a meaningful increase in collagen after one or two treatments, though additional treatments are recommended to offset the natural aging process. For those in their 30’s-40’s, once a year Density treatment is recommended for “collagen banking” to maintain youthful levels of collagen in the skin. For those who are 45+, Density treatments 2-3 times a year is recommended for rejuvenation and a more youthful, lifted appearance.
Is Density appropriate for all skin types?
Yes! Density works great on all skin types and uses real-time skin sensing to adjust the treatment to your skin’s unique needs.
